I have a business form that we use pretty often and its getting pretty tired. I got it years ago and its simply a scanned in doc that we got somewhere and tweeked on over the years. Basically I just need to start over and make a new one. Not sure what is the best way, Adobe or Word, but I need someone that has skills in this stuff to re-make the form, and incorporate some minor edits. The old version is attached. Not a problem paying for someone's time to do this.

I will also need your company logo preferably in a .ai (Adobe illustrator) format if you have it. A .jpg would work if it's all you have.

I have recreated many forms here at work.. Always a positive use of creative options

Scan it with a software called omniform ( maybe omnipage ) and whalla out comes a fborm.
